Home and Community-Based Services (HCBS)
Supporting Independence Where It Matters Most
All of the services we provide at The Blooming Homes fall under Arizona's Home and Community-Based Services (HCBS) framework. HCBS is designed to give individuals with disabilities the support they need while keeping them in home and community settings, rather than institutional ones.
Through the Arizona Division of Developmental Disabilities (DDD), we are authorized to deliver three core service types:
Habilitation
Our Habilitation services include both Adult Developmental Homes and Group Home settings. These residential programs provide 24-hour care, individualized service plans, and structured skill-building activities. The focus is on helping each person develop the daily living, social, and self-care skills they need to live as independently as possible.
Respite Services
Respite gives primary caregivers a temporary break while their loved one receives safe, supervised care from our trained staff. Respite can be planned in advance or arranged on a shorter timeline depending on the situation. It's a critical support for families, and it gives the individual an opportunity to experience a new environment with caring, familiar staff.
Attendant Care
Attendant Care provides direct, one-on-one support to individuals in their daily activities. This can include help with personal hygiene, meal preparation, community outings, errands, and other tasks that keep the individual active and engaged. Attendant Care is delivered based on the individual's ISP and tailored to their specific routine and preferences.
