Group Home Habilitation

Our group homes provide around-the-clock care and supervision for individuals who need a higher level of daily support. Every resident receives a comprehensive assessment when they enter the program, and from that assessment, we develop an Individual Service Plan (ISP) in partnership with the individual and their family.

These plans guide everything: training schedules, skill-building activities, health management, and progress tracking. The goal is always to move toward greater independence.

Our residential staff are trained and experienced in the following areas:

• Medical and counseling appointment coordination
• Medication management
• Individual Service Plan (ISP) implementation
• Behavior Treatment Plan (BTP) support
• Transportation for appointments and community activities
• Personal care assistance
• Social skills development
• Problem-solving and daily living skills

Each home is equipped with safe, accessible vehicles so residents can get to appointments, errands, and community outings reliably.
